Registered Nurse and Case Manager (February 2015 to July 2016)

 with nursing staff and provided TB testing services to TB outreach clinics throughout Franklin County.  I aided in the prevention and control of TB disease through education and assisting with investigations.  I worked closely with my healthcare team to assist with the prevention and control of TB disease in direct observation therapy team.  I assisted to provide professional nursing care for perinatal, pediatric, and adult patients with ages ranging from 0-98 years of age.  I assisted nursing teams in performing triage, patient assessment, history, vital signs, and procedures for specimen collection.  I administered intradermal injections & monitored daily medication administration and performed outpatient procedures at the direction of the provider.  I assisted to maintain open communication with patients concerning their treatment progress and diagnostic reports in conjunction with nursing staff.  When needed I Identified and coordinated referrals to community resources for patients.  I also reported and assisted with coordinating care with multidisciplinary teams as necessary.  I also performed all functions necessary to assure clinic flow and patient care accurately and efficiently.  I independently or in conjunction with staff, conducted mandated TB screenings, and special education assessments in community setting treating a large number of patients.  I ensured external follow-up where required.  I provided education to all age levels on TB including prevention, current treatments and screening techniques as directed by state and federal guidelines.  I assisted with physicians office appointments and efficiently used both Medical Manager and Citrix programs to ensure proper clinic functioning.  I reported patient assessment findings and administered medications daily in compliance with current State and Federal guidelines.  I assisted in counseling and educating patients about reproductive health issues and wellness.  I gained expertise in co morbidities of STDs, HIV, Hepatitis, and family planning issues to assist patients while they were receiving active TB treatment.
